About the Plan
The Premium Plus option combines Medicare coverage with active worker Concordia Health Plan coverage. Because Medicare becomes the employee's primary coverage, the Premium Plus Option provides additional coverage at a reduced contribution rate for you. Win-Win!
You must have fewer than 20 full- and part-time employees on payroll to qualify. Other restrictions apply.
If you are an eligible employer, your employee and/or their enrolled spouse must be 65 or older and an active LCMS employee to qualify for this opportunity. Other restrictions apply.
Premium Plus provides enrolled workers with coverage for medical, vision, dental, hearing, and prescription drugs. Members can also participate in Vitality, have access to the Employee Assistance Program, receive Medicare Part B premium reimbursement and can utilize Livongo Diabetes Management and Naturally Slim Weight Loss programs.
