Remember: You decide who receives your benefits! By designating your beneficiaries, you control who receives your benefits when you die. Log into your Member Portal to designate your beneficiaries.
What to do if a loved one dies
When you lose a loved one there is a lot to deal with, from the emotions of your loss to piles of paperwork – and reporting the death to places that need to know. At Concordia Plans, we’ve tried to make that process as simple as possible by providing two ways you can report the death of a member - call our Survivor Benefits team at 888-927-7526, or complete the online form to initiate the claims process.
Basic Life Insurance
Basic life Insurance is an important benefit if providing for your family after your death is important to you. When you’re enrolled in the Concordia Disability and Survivor Plan, you have Basic Life Insurance benefits that includes you, the worker and additional life benefits for your spouse and children.
How it works:
While your Basic Life Insurance benefits provide robust coverage, you may need to purchase additional insurance. Supplemental Life Insurance allows you to purchase additional term life polices for yourself, and your spouse or children. Accidental Death & Dismemberment insurance helps provide additional benefits if you are seriously disabled or die due to an accident. Accident Insurance and Critical Illness help protect your family for medical expenses due to emergencies.
