If your ministry previously used the ACR Online Tool, please note that this tool is not available for 2024 submissions. Instead, ACR forms were mailed to your ministry. Please complete and return the ACR form by Jan. 31, using the envelope provided. If you prefer to complete a digital template with your workers’ 2024 salaries, please contact your Account Manager to request a digital form and instructions. Share these items with new and current workers in 2024
If you offer the Concordia Health Plan, be sure to share these legal notices and documents with your workers in 2024. You can find these at ConcordiaPlans.org/Plans.
As an employer, it is your obligation to provide the following notices and documents to your new workers:
For all employees regardless of CHP eligibility:
Marketplace Notice – should be provided within 14 days of the worker’s start date.
Version for employers who offer a health plan to some or all employees.
Version for employers who do not offer a health plan.
Children’s Health Insurance Program (CHIP) Notice.

Please note that Concordia Plans provides all these notices prior to Open Enrollment. So, you only need to provide these notices to current workers who increase their hours and become eligible for CHP coverage during the plan year outside of the Open Enrollment period.
2024 updates to the Concordia Disability and Survivor Plan
The disability benefits you offer to your workers through the CDSP provides them with peace of mind, knowing that they will have income replacement during times of need. Here’s a review of updates that were made to the Plan as of Jan. 1, 2024.
Elimination Period Update – amount of time between when disability begins and when disability benefit payments begin.
Seven-day elimination period (benefits begin on the eighth day from the date of disability).
Healthcare Coverage Update (for those enrolled in the Concordia Health Plan):
Members and all enrolled dependents will continue coverage in the CHP with contributions waived beginning at the fourth month.
Monthly stipend beginning the first Jan. 1, following the seventh month of disability.
Example: Disability begins March 1, 2024, stipend begins Jan 1, 2025
TIP: We recommend reviewing your Employee Handbook to ensure your current PTO policy reflects the update to the elimination period.
It’s W-2 time!
As an employer, it’s your responsibility to prepare W-2 forms and provide these documents to employees on or before Jan. 31. Additional copies must be sent to the Social Security Administration so employees' tax returns can be verified.
